The Lead Case Manager is responsible for coordinating case management, training new case managers, and supervision to work of other case managers. They ensure contractual requirements and client expectations are met at the on-site residential facility. Reports to Site Manager.

DUTIES AND RESPONSIBILITIES:

  • Coordinates case management services for enrollees
  • Ensures case managers initiate and maintains contact with each enrollee
  • Provides oversight of case managers, ensures quality control over work, and ensures adherence to contract policies and procedures
  • Provides oversight of Case Managers, responsible for team building, coaching, staff appraisals, and performance measures
  • Ensures assessments and screenings are conducted in accordance with contract policies and procedures
  • Provides timely and accurate reports
  • Works closely with counselors and care coordinators to ensure communication
  • Maintains the CM Database System in accordance with contract policies and procedures
  • Other duties as assigned by supervisor

OTHER DUTIES:

  • Performs duties in a safe manner.
  • Follows the corporate safety policy.
  • Participates and supports safety meetings, training and goals.
  • Ensures safety and security of all UAC and the operating conditions within area of responsibility.
  • Maintains a clean and orderly work area.

QUALIFICATIONS:

  • Bachelor's degree in behavioral sciences, human services, or social services fields from an accredited institution
  • 3 years of progressive employment experience in aforementioned fields that demonstrates supervisory and case management experience
  • 1 years of supervisory experience
  • Fluent in English and Spanish (speak, read, and write)
  • Clean criminal background check
  • Meet the requirements of the contract for all immunizations
  • Must be able to multi-task, be detail-oriented, be organized and have excellent verbal and communication skills

PREFERRED QUALIFICAITONS:

  • Master's degree in the behavioral sciences, human services or social services fields
  • 3 years of supervisory experience (reduce to 6months only since bachelor's is 1 year)
  • Child welfare experience and professional licensure

PHYSICAL REQUIREMENTS - only for on-site assignments:

  • Ability to ascend/descend stairs
  • Ability to lift up to 15 lbs.
  • Work involves sitting and standing for prolonged periods of time.
