Overview

Shenandoah Oncology is in search of an LPN Navigator to serve as a knowledgable and compassionate guide for patients. The LPN Navigator is responsible for providing individual assistance to patients, families, and caregivers to overcome health care system barriers and provide timely access to quality healthcare within a community oncology setting. The LPN Navigator is under general supervision of an RN, by providing patient education, assist with facilitation of access to high quality health and psychosocial care through all phases of the cancer continuum. Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging, dietary, and others. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process. The position is Monday - Friday, 8:00 - 4:30 p.m. ET., and our practice observes several paid holidays and offers a competitive benefits package.



Responsibilities

  • Provides coordination of care during the full continuum of cancer care experience.
  • Observes, records, and reports patient's condition and reaction to drugs and treatments to physicians.
  • Screens patients for appropriate information. Instructs patients in collection of specimen samples for testing.
  • Responds to and refers incoming telephone calls. Instructs patient and family regarding medications and treatment instructions.
  • Phones in prescriptions and makes other patient calls as required or directed by physicians.
  • Maintains patient files, records, and other information.
  • Participates in team meetings led by direct supervsior.
  • Provides education materials.
  • Supports patient advocacy and empowerment.
  • Participates in community outreach and cancer prevention education.


Qualifications

  • High school graduate or equivalent.
  • Graduate from an accredited school for practical nursing.
  • This position requires at least five years of experience as a practical nurse.
  • Current LPN state license and registration with the State Board of Practical Nursing in VA.
  • Current BCLS Certification required within 3-months of hire.
