The Post-Acute Care Specialist acts as a liaison between hospitals and post-acute care centers for smooth patient transfers to Prestige centers. Develops, implements, and maintains reporting systems and data analyses to drive growth of hospital referral program. You are known here. In this job, every day you will: * Educates patients on Prestige services and provides an efficient transition to post-acute centers. Coordinates with Prestige centers to ensure appropriate and timely placement of patients. * Manages patient transfers to Prestige centers by coordinating physician orders, medical equipment, medications and required documentation. * Evaluates patient financial and insurance resources including, but not limited to, Medicaid, Medicare, HMO and private insurances and make recommendations for placement for appropriate utilization. * Manages and tracks key program metrics for Prestige and hospital. * Collaborates with Prestige centers to develop and maintain systems to evaluate a patient's risk for hospital readmission and communicates recommendations on patient specific interventions. * Partners with hospital case managers and Prestige centers to prevent unnecessary readmissions back to the acute setting. Reports on Return to Hospital (RTH) rate. * Creates and maintains reports on Centers for Medicare and Medicaid Services (CMS) and the Center for Medicare and Medicaid Innovation (Innovation Center) payment models. * Tracks referrals and Prestige responses, admissions to Prestige from hospitals, and all denials by Prestige by discharge, length of stay, etc. Education experience you need to qualify: * Minimum of one year of experience in a health care environment with a working knowledge of Medicare, Medicaid, Insurance funding systems required. * Bachelor's Degree in business administration or equivalent knowledge and skills obtained through a combination of education, training, and experience. * Must be detail-oriented and able to work in deadline driven situations. * Excellent time management, organizational, and critical thinking skills. * Experience in transition from hospital to skilled, assisted, or acute care settings preferred EOE/M/F/VETS/DISABLED - At Prestige, it is our promise to personally touch lives every day. SNF/PCI Hourly: Full-time and Part-Time team members (over 20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.:::
